Command Code adds OpenAI Responses API support for 98 models

Command Code now supports the OpenAI Responses API. Requests use POST https://api.commandcode.ai/provider/v1/responses and can reach 98 open and closed models.

Function and custom tools remain client-side. Built-in tools are passed through only when the selected model supports them.
